Warranty Service Request Form

URI Eagle Ceramic Knives offers a free sharpening service to repair minor damage to the tip or cutting edge of our knives. This free service is available to all original purchasers of our products during the valid 5-year warranty period. There is a $10 shipping and handling fee; which must be sent as a check or money order with the package containing the knife/knives.

To request our free warranty service please provide the following information and mail to us at the address below along with the knife or knives (up to 3 knives can be sent together for one $10 shipping and handling charge):

· Customer’s Full Name (First & Last name, please…)

· Customer’s Street Address or PO Box

· Customer’s City, State, and ZIP-code

· Phone Number with Area Code

· Location & Date of Purchase (or attach copy of your Proof of Purchase)

Important! Please pack your knife returns very carefully – use your knife’s original packaging if possible!
